I've been going around in circles trying to discover why my server is no longer spectating people who false-start? It gives out 30 and 45 second penalties but nothing after. Has anyone encountered this?

Are you using an InSim application (Airio, LFSLapper)? Airio does have autospec on jumpstart, but I think you need to configure that.
I'm not sure if LFS without any add-ons spec people on jump start - it would just give DT or SG afaik. 30s or 45s Pentalty.

Yeah I'm using Airio - the odd thing is that a demo server on the same install (and with the same settings works correctly.
I read in the Airio thread that the auto-spec is actually handled by LFS; not sure if it was correct.

Have a look at your Airio.srv:
Have a look at your config (open it up, CTRL+F and enter "false start" to locate it faster!)
Have look at other .srv files, you might have another file to handle your demo server.

I couldn't find the line in any of my srv files at all. Even from way back. I guess maybe I removed it at some point.

While you're here - any idea why I can't use the !remove command? I get a message in purple text "insufficient rights" - even when doing it from the console. It's not listed in the Manual and a Pro command.
Are you logged in as admin? Maybe your Licence name need to be entered into the Airio.cfg file as a Limad for the !remove to work. Sorry, Im not an admin, and not aware of the !remove command.
